Skilled Nursing

The Cerenity Care Center – Humboldt offers round-the-clock skilled nursing and rehabilitation therapies for up to 125 residents. Skilled nursing is the highest level of care a person can receive outside of a hospital. Long-term care, short-term rehabilitation and hospice care are provided on Humboldt’s second and third floors. The fourth floor is a secured area for residents with memory loss. The majority of rooms are shared; however, there are 39 private rooms.

The Care Center has a private Comfort Care Room that allows residents and their families to spend their final days together.

Prior to admission, a social worker and nurse conduct an assessment to determine the optimum level of care for each individual resident. Our social services department assists incoming residents and their families with all aspects of moving in and adjusting to life at Humboldt.

Throughout a resident’s stay, social workers ensure each resident’s needs are met; assist with applying for financial assistance; and refer residents and family members to community resources as needed. They also assist with discharge planning as appropriate.

• Resident Choice Meal Plan, an innovative meal plan created by Cerenity Senior Care. Guest meals available for a fee.
• 24-hour nursing
• Medication administration and monitoring
• Regular assessment/evaluation to monitor changes
• Assistance with bathing, dressing and grooming
• Daily housekeeping of room
• Personal laundry service for clothing, bed linens and towels
• Recreation and social activities, such as social events, craft groups, music programs and theme parties, plus outings to destinations
• Spiritual care, including weekly non-denominational worship, Bible study, Mass and rosary
• All utilities except telephone
• Senior satellite TV with basic cable channels
• Wireless Internet access
• Personal trust account; residents may deposit personal funds and withdraw money when needed

Services available at additional cost, which may be covered by insurance

• Services of a specialist or physician by personal request or by physician order
• Dental, podiatry, psychiatry, ophthalmology and audiology services on-site through contracted professionals
• Prescription medications and medical supplies
• Purchase or rental of specialized medical equipment.

• Physical, occupational and speech therapy
