REHAB THERAPY – RPT, PTA, OT, COTA, SPEECH THERAPIST

• Occupational Therapist - on call – weekends.

• Occupational Therapist Assistant- positions: -on call – weekends.

• Speech Therapist – positions: -on call – weekends.

Inhouse program to include coverage for LTC & TCU.  Schedules vary dependent on staffing levels to support services FT, PT & casual/on-call.

Patient treatments to support rehab of LTC & TCU patients both inhouse and outpatient programs

Education and licensure to ensure competency. LTC or TCU experience preferred.

RN- HOME CARE

1 @ 32 hours    9:00 – 5:30

Performs assessments on tenant and reassessments as per policies and established practices.   Develops and implements appropriate plan of care based on assessment and diagnosis tailored to meet each individual’s needs (physical, cultural and psychosocial), evaluates plan of care and makes necessary changes, works to meet individual tenant needs.   Demonstrates knowledge of medications and their correct administration based on tenant and their clinical condition.  Evaluates tenant responses to medications.   Plans for and provides tenant, family and caregiver education, identifies learning needs, clearly communicates information and documents tenant response to education.  Documents according to current standards and policies.   Plans for tenant discharge, anticipates and plans for tenant needs, collaborates with the team when appropriate and assesses the tenant prior to discharge.

Current state licensure as a Registered Nurse, which must be in good standing.  CPR certified.  Must have good communication skills.  Previous Home Care experience helpful.

RECREATION AIDE

Every other week end  8 – 4:30

M-F 1:00 pm – 7:00 pm

Assist and/or lead assigned group, small specialized groups or one to one programs.  Assist residents to and from programs. Maintain/record daily attendance of each resident.4. Implements Plan of Care.

Have knowledge of various resident activities. Be able to work in close cooperation with other departments.  Ability to communicate effectively, verbally and in writing.  Dependability, punctual, flexible, enthusiastic, creative and a desire to work with elders. Compassion, sensitivity to the needs, interests and abilities of residents.

DIRECTOR OF REHABILITATION

Fulltime – M-F

Responsible for overseeing and directing rehab therapy staff and services on daily basis. Ensures department compliance, financial goals, and patient care standards.

Registered therapist (RPT, OT or Speech) with knowledge of TCU & LTC standards and routines.

LPN – MEMORY CARE ASSISTED LIVING

• 1 @ 32 hours ppd 6:30 am to 3:00 pm – includes everyother weekend,
• 1 @ 40 hours ppd 2:30 pm to 11:00 pm – includes everyother weekend

A clinically competent, licensed practical nurse who provides care to a group of tenants under general supervision/delegation of a Registered Nurse according to established policies, procedures, protocols and standards of practice and care.  Effectively interacts with tenants, families, responsible party, and all health team members. On-call every other week.

Qualifications for this position include: current LPN licensure in the State of Minnesota.  CPR certified, and must have good communication skills. Previous Assisted Living/Home Care experience preferred.

MAINTENANCE

10:00 am to 7:00 pm

Responsible for providing maintenance for the building, physical plant, and equipment insuring proper repair and operations. Provides for the safe and efficient operating, maintenance, and repair of boilers, heating and air conditioning equipment, repair of building and other mechanical equipment. Maintains building emergency systems. Provide necessary report preparation, record keeping, and operating logs. Completes work orders as necessary.

Two years vocational technical training in building maintenance or five years related experience. A minimum of a special class boilers license.   Knowledgeable and skilled in operating building mechanical systems including electrical, plumbing, heating, air conditioning and pneumatics. Previous long term care experience. Be able to work in close cooperation with other departments. Ability to communicate effectively, verbally and in writing.
