RECREATION AIDE

M-F 1:00 pm – 7:00 pm

Assist and/or lead assigned group, small specialized groups or one to one programs.  Assist residents to and from programs. Maintain/record daily attendance of each resident.4. Implements Plan of Care.

Have knowledge of various resident activities. Be able to work in close cooperation with other departments.  Ability to communicate effectively, verbally and in writing.  Dependability, punctual, flexible, enthusiastic, creative and a desire to work with elders. Compassion, sensitivity to the needs, interests and abilities of residents.

RESIDENT ASSISTANT

16 hours per pay period

Day shift

The Resident Assistant (RA) will provide patient/resident care as required under the direct supervision of the licensed nurse.  The Resident Assistant completes assigned tasks to ensure timely, efficient and safe patient/resident care.  Communication and interactions with tenants, families and other health team members is an essential component of this position.

Must have good communication skills; must be able to read, write and communicate in English; have basic computer skills or willingness to acquire during orientation. Previous Assisted Living/Home Health, and/or Registered Nursing Assistant experience preferred.

THERAPEUTIC RECREATION COORDINATOR

Responsible for resident activities programming. Includes developing, planning and facilitating daily resident leisure activities.

Bachelors degree in recreation or completion of accredited vocational training program, and/or equivalent experience.

RN- HOME CARE

1 @ 32 hours    9:00 – 5:30

Performs assessments on tenant and reassessments as per policies and established practices.   Develops and implements appropriate plan of care based on assessment and diagnosis tailored to meet each individual’s needs (physical, cultural and psychosocial), evaluates plan of care and makes necessary changes, works to meet individual tenant needs.   Demonstrates knowledge of medications and their correct administration based on tenant and their clinical condition.  Evaluates tenant responses to medications.   Plans for and provides tenant, family and caregiver education, identifies learning needs, clearly communicates information and documents tenant response to education.  Documents according to current standards and policies.   Plans for tenant discharge, anticipates and plans for tenant needs, collaborates with the team when appropriate and assesses the tenant prior to discharge.

Current state licensure as a Registered Nurse, which must be in good standing.  CPR certified.  Must have good communication skills.  Previous Home Care experience helpful.

RESIDENT ASSISTANT

36 hours per two week pay period

Evening shift

The Resident Assistant (RA) will provide tenant care as required under the direct supervision of the liscensed nurse. The RA completes assigned tasks to ensure timely, efficient and safe tenant care. Communications and interactions with the tenants, families and other health team memebers is an essential component of this job.

Must have good communication skills; must be able to read, write and communicate in English;have basic computer skills or be willingness to aquire durieng orientation. Previous experience in Assisted Living/Home Health, and/or Registered Nursing Assistant experience preferred.

Nursing Assistant

Daily care of residents and patients within established practices including all activities of daily living.

FUND DEVELOPMENT / VOLUNTEER DIRECTOR

 Cerenity Senior Care Humboldt Campus is currently seeking a creative, energetic, and organized candidate to join our team as the Fund Development/Volunteer Director. This individual will launch our new philanthropic effort to cultivate relationships for acceptance and recognition of outstanding senior care within our community, and coordinate our on campus volunteer program.   

Primary responsibilities will include:

• Coordination of activities and events, for annual and major gift fundraising
• Create and advance philanthropic giving to Cerenity Senior Care Humboldt Campus through ensuring a diversified and stable base of funding from all sources
• Spearhead donor mapping and research through follow-up calling and mailings to build strong donor relationships and enhance donor confidence in the organization.
• Communicate Cerenity’s compassionate mission and values to donors for continued support of the elder population within our community
• Oversee acquisition, solicitation, stewardship and monitoring of all donor data and relationships.
• Conduct and coordinate the preparation and execution of donor proposals, associated with various donor sources.
• Other duties associated with fund development, major gifts, and in-kind development as assigned.

Qualifications required of preferred candidate include strong relationship building skills, a passion for sustainable development of the foundation, and proven success working with potential donors / volunteers. Excellent communication skills (verbal & written), positive attitude, unparallel leadership skills, and the ability to work in a collaborative manner are needed. Experience in public relations, communication, marketing, campaigning, and/or event planning is preferred. Previous experience in recruiting and directing volunteers is a plus!

Director of Rehabilitation

Full time/80 hrs pre 2 week pay period

Business hours; Monday through Friday

Responsible for overseeing and directing rehab therapy staff and services on daily basis. Ensures department compliance, financial goals, and patient care standards.

Registered therapist (RPT, OT or Speech) with knowledge of TCU & LTC standards and routines.

COOK

40 hours ppd 9:00 am -5:30 pm and 11:00 am – 7:00 pm

Responsible for preparing and serving hot and cold foods for all customers using standardized recipes and proper food handling techniques.

Minimum one year experience and training in quantity food preparation. Ability to lift up to 50 lbs. Ability to read, write and speak English. Demonstrated ability to follow written and oral directions including recipes, production sheets, etc. Knowledge of large kitchen equipment and how to use equipment properly. Knowledge of proper food handling and sanitation techniques.   Experience must include meat cookery, sauces, soups, vegetables and baking.

Responsible for delivery of quality resident care in accordance with established standards of practice. Duties include medication administration, treatments, assessment, interventions and documentation of resident medical cares. Oversees and directs nursing assistants for activities of daily living in a safe environment.

Must be able to identify, prioritize, coordinate, communicate and assign work for self and others. Must be able to support elders, family members and others ensuring a compassionate and caring atmosphere using strong customer service skills. Requires graduation from an accredited nursing program and current Minnesota LPN license in good standing.

NURSING ASSISTANT

Responsible for resident care, duties as assigned. Assist in meeting physical, social and emotional needs of the residents.

Qualified applicant must posses at least six months experience in skilled healthcare setting. Completion of accredited nursing assistant training program and current registration with the Minnesota Department of Health.

NURSING ASSISTANT

• 3 @ 15 every other weekend, 1 @ 22.5, 1 @ 30 ~Days 6:30 am – 2:30 pm,

LPN – MEMORY CARE ASSISTED LIVING

• 1 @ 32 hours ppd 6:30 am to 3:00 pm – includes everyother weekend,
• 1 @ 40 hours ppd 2:30 pm to 11:00 pm – includes everyother weekend

A clinically competent, licensed practical nurse who provides care to a group of tenants under general supervision/delegation of a Registered Nurse according to established policies, procedures, protocols and standards of practice and care.  Effectively interacts with tenants, families, responsible party, and all health team members. On-call every other week.

Qualifications for this position include: current LPN licensure in the State of Minnesota.  CPR certified, and must have good communication skills. Previous Assisted Living/Home Care experience preferred.

ADULT DAY SERVICES AIDE

casual

hours – 7:30 am – 4:30 pm – flexible

Work with and assist Director with operation of Adult Dayaway program

6 months previous experience working with elderly and/or individuals with disabilities in an Adult Daycare or Long-Term Care setting.  Good communication and organization skills.  Ability to lift a minimum of 50 pounds.  Ability to withstand prolonged walking, standing, bending and stooping while working with participants.
