Establish and maintain meaningful, appropriate, professional relationships with clients so that the social worker may effectively assist the client in attaining his/her goals. Serve as client advocate. Function effectively as part of an interdisciplinary team responsible for client care.
Bachelor’s degree in Social Work. 3-5 years of experience working with the elderly in a long term care setting. MN license to practice Social Work.

To serve food, stock supplies, provide service to all customers and/or perform sanitation tasks that relate to food preparation and service.

Supports Cerenity Social Service department filling in for vacations and leaves of absence. Duties include admission, discharge, MDS documentation, care plan, and interdisciplinary team communications and processes.
Graduate of accredited social work program and current Minnesota LSW in good standing. Prefer experience in nursin home setting with knowledge of MDS. Will consider new graduate if posesses exposure to healthcare setting.

Responsible for providing maintenance for the building, physical plant, and equipment insuring proper repair and operations. Provides for the safe and efficient operating, maintenance, and repair of boilers, heating and air conditioning equipment, repair of building and other mechanical equipment. Maintains building emergency systems. Provide necessary report preparation, record keeping, and operating logs. Completes work orders as necessary.
Two years vocational technical training in building maintenance or five years related experience. A minimum of a special class boilers license. Knowledgeable and skilled in operating building mechanical systems including electrical, plumbing, heating, air conditioning and pneumatics. Previous long term care experience. Be able to work in close cooperation with other departments. Ability to communicate effectively, verbally and in writing.
A clinically competent, licensed practical nurse who provides care to a group of tenants under general supervision/delegation of a Registered Nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with tenants, families, responsible party, and all health team members. On-call every other week.
Qualifications for this position include: current LPN licensure in the State of Minnesota. CPR certified, and must have good communication skills. Previous Assisted Living/Home Care experience preferred.
