HealthEast expands medical directorships at all Cerenity campuses

Mark MacDonald, MD, medical director for HealthEast Medical Care for Seniors, has been named medical director for all of Cerenity Senior Care’s post-acute facilities.

Medical Care for Seniors provides a team of doctors and geriatric nurse practitioners to make visits to post-acute facilities – transitional care units (TCUs), skilled nursing facilities (SNFs) and assisted living facilities (ALFs) – in our region. HealthEast has been delivering care on Cerenity Senior Care campuses for the past several years. Dr. MacDonald’s new medical directorship role moves the HealthEast/Cerenity collaboration to the next level.

Dr. MacDonald will bring the physician leadership voice to the table in senior executive leadership’s strategic planning process; he will attend monthly executive meetings to offer medical perspective on a number of issues including enhancing the quality of care, helping to reduce hospital re-admissions, improving provider-to-provider communications across the continuum and improving patient/family satisfaction. Dr. MacDonald will also partner with operational and clinical leaders at all the sites to develop and implement initiatives which improve clinical quality outcomes. He will have offices at every campus and will be visiting sites on a weekly basis. Cerenity Senior Care offers three campuses in the area: Humboldt (30 TCU beds, 117 AL units); White Bear Lake (40 TCU beds, 173 SNF beds, 50 AL units); and Marian of St. Paul (24 TCU beds, 90 SNF beds and 172 AL units).

“Bethesda Hospital and Cerenity have been closely working together on several quality initiatives for the past two years,” said Rahul Koranne, MD, executive medical director, Bethesda Hospital and Community Services. “It is exciting that we can now provide onsite medical leadership and support to our patients and families at Cerenity facilities through Medical Care for Seniors and further strengthen the post-acute section of HealthEast’s continuum of care.”
